Through our work with agencies, we have found that the documentation submission list is a MUST when submitting your documentation. It serves two purposes: 1. It helps your agency to stay organized and stay on task as an internal tool that will provide a guide for you when you're gathering and organizing documents. It ensures that everything's in a clean and organized format. 2. The document list provides the reviewer a guide of all of the documents that you are submitting.
The document list is a blank template. We've taken all of the required documents that you will need to include in your submission and we've put them here in order under three categories: technical documentation, your claim period review and miscellaneous documents. Using this list as a guide, you will be able to gather the documents, upload and merge in this particular order. Next, you will fill in your document list with dates. A sample has been provided for you. In this example, patient John Doe has a claim period of 1/1/22 - 5/31/22. Therefore, each claim number will be listed by DOS and will have Orders, POC/IDG review, Assessments, and visit notes. We include the dates for each document. It gives the reviewer that extra detailed organization and intention.
